Starch safety in resuscitation.

چکیده
The Western Cape Department of Health (WC DoH) has taken a decision to withdraw all intravenous fluids (IVFs) containing hydroxyethyl starch (HES) from hospitals in the Western Cape, with similar action contemplated in the Free State and Gauteng. This was in response to recommendations from: • The European Medicines Agency’s Pharmacovigilance Risk Assessment Committee (EMA PRAC) that HES IVFs be withdrawn from clinical use. • The United Kingdom Medicines and Healthcare Products Regulatory Agency (MHRA) which has issued a recall of all HES IVFs in the UK. • The United States Food and Drug Administration which advises that HES IVFs be used with caution in ICU, cardiac surgery and patients with known kidney disease or coagulopathy. Further advice was that HES should be stopped if coagulopathy or renal dysfunction develops, as well as that renal function should be monitored for 90 days after HES administration.

CARING FOR THE CRITICALLY ILL PATIENT Association of Hydroxyethyl Starch Administration With Mortality and Acute Kidney Injury in Critically Ill Patients Requiring Volume Resuscitation A Systematic Review and Meta-analysis
FLUIDS ARE A CORE ELEMENT IN the resuscitation of critically ill patients and the relative superiority and safety of different resuscitation solutions has been the focus of considerable debate. Crystalloid solutions are inexpensive and readily available, while colloid solutions may minimize resuscitation volumes, and may sustain intravascular volume for longer durations. Conflicting results fro...
متن کاملFluid-induced coagulopathy: does the type of fluid make a difference?
Crystalloid and colloid solutions are used for resuscitation of the critically ill. One set of options, widely used today, are different preparations of hydroxyethyl starch (HES). However, the safety of HES regarding impairment of blood coagulation remains incompletely elucidated, a circumstance that limits its clinical use. Understanding mechanisms and potential differences between low-molecul...
متن کاملRespiratory Review of 2013: Critical Care Medicine
Several papers on respiratory and critical care published from March 2012 to February 2013 were reviewed. From these, this study selected and summarized ten articles, in which the findings were notable, new, and interesting: effects of high-frequency oscillation ventilation on acute respiratory distress syndrome (ARDS); safety and efficacy of hydroxyethyl starch as a resuscitation fluid; long-t...
متن کاملStarch safety in resuscitation - when will we ever learn?
Recent trials have failed to demonstrate a survival benefit from the use of hydroxyethyl starches (HES) as a colloid in fluid resuscitation and have raised concerns of renal harm. In severe sepsis, there is a concerning signal of increased mortality. New high-quality systematic reviews consistently demonstrate a statistically non-significant relative risk of death of 1.08 - 1.10 and a significa...
متن کاملPhase II trial on the use of Dextran 70 or starch for supportive therapy in Kenyan children with severe malaria.
OBJECTIVES A previous meta-analysis has shown a consistent survival benefit in children with severe malaria receiving human albumin solution compared to other resuscitation fluids. Human albumin solution is expensive and not readily available in Africa. We examined the safety and efficacy of the fluid resuscitation with two synthetic colloids, Dextran 70 and hydroxyethyl starch, to inform futur...
متن کاملVertebral Heart Score: A Useful Parameter in Determination of Resuscitation Endpoint in Dogs with Hemorrhagic Shock
BACKGROUND: GReliable prediction of patient response to fluid resuscitation is a crucial issue in the management of hemorrhagic shock. OBJECTIVES: This study was designed to investigate vertebral heart score (VHS) variation in fluid resuscitation of hemorrhagic shock patients and the feasibility of this method in determining resuscitation endpoint. METHODS: VHS values were assessed using previ...
